Tetrofosmin, a bisphosphine complexed with technetium-99m, is utilized for imaging to detect myocardial ischemia, myocardial infarction, and evaluate left ventricular function. Technetium-99m tetrofosmin is a positively charged complex with tetrofosmin serving as the functionalized ligand.
Upon intravenous injection, technetium-99m tetrofosmin quickly gathers in the myocardium and various non-target organs such as the liver, kidney, spleen, and skeletal muscle. The initial pass extraction fraction in the myocardium is 0.45, with minimal clearance at three hours post-injection and a typical myocardial uptake of up to 1.2% of the injected dose. There is more rapid clearance of the tracer from the myocardial wall during periods of stress compared to rest, efficient clearance from the blood, and swift washout from non-target organs at peak exercise.
Furthermore, there is notable accumulation of technetium-99m tetrofosmin in skeletal muscle and increased retention of the agent during physical activity. Less than half of the administered contrast agent is excreted through the kidneys (around 40%) and intestines (approximately 34% and 25% at rest and during exercise, respectively) within 48 hours post-administration, regardless of activity level.
Myocardial perfusion imaging (MPI) using technetium-99m tetrofosmin is among the favored radionuclides for MPI. In clinical settings, it is a commonly used diagnostic agent for assessing left ventricular dysfunction and myocardial viability, employed both independently and in conjunction with modalities that measure myocardial metabolism.
